Facility Compliance Risk. Quantified.
FDA’s dashboard shows you one plant, in one agency, at a time. We show you which of your suppliers just became a systemic risk — across FDA, EPA, and OSHA, in one alert feed. The only facility risk score that spans all three federal regulators, recomputed nightly.
Coverage
100,000+ FDA-inspected facilities across food, drug, device & biologics · repeat severe-violation flags · facility scores recompute nightly · webhook alerts when your vendors land a new violation
Three Agencies. One Risk View.
FDA, EPA, and OSHA are authoritative public sources — but each lives in its own portal. We normalize and connect them into a single facility-level view, scored and monitored.
FDA Enforcement & Recalls
All plansFDA inspections, classifications (NAI / VAI / OAI), Form 483 citations, recalls, and enforcement actions across 100,000+ inspected food, drug, device, and biologics facilities — the live FDA signal we normalize and join to EPA and OSHA.

Repeat Offender Flags
Insurer+Flags facilities with a repeat pattern of severe regulatory violations across our FDA, EPA, and OSHA coverage — the systemic bad actors, not one-off citations. (Per-facility cross-agency linkage rolling out.)

What Compliance Teams Do With the Data
Real workflows our subscribers use every day.
Underwrite Facility Risk Before Binding Coverage
Pull a facility's full FDA / EPA / OSHA history, sub-score breakdown, and peer-industry comparison. Decline, surcharge, or refer based on objective signals — not last quarter's loss runs.
Surface Repeat Offenders
Find facilities flagged for a repeat pattern of severe violations — the systemic bad actors, not one-off citations. Filter by NAICS, state, or 3-year trend.
Monitor Your Supplier Roster
You can't set an alert on a 483. Upload your vendor roster once; we watch FDA, EPA, and OSHA and webhook you the moment any of them slips — a fresh recall, ECHO action, or OSHA citation. Cut letter-of-credit and continuity-of-supply risk.
Quantify Compliance Exposure for Diligence
For an M&A target or PE portfolio: aggregate facility-level risk scores into a portfolio score. Compare against industry NAICS benchmarks. Flag unresolved willful violations.

Frequently Asked Questions
What agencies do you cover?
We cover three federal regulators: FDA (inspections, classifications (NAI / VAI / OAI), Form 483 citations, recalls, and enforcement actions across 100,000+ inspected facilities; warning letters rolling out), EPA (ECHO enforcement actions, Clean Air / Water / RCRA), and OSHA (workplace inspections, citations, abatement — back to the 1970s).
Isn't this already on FDA's free dashboard?
FDA's site is excellent for looking up one facility's FDA history. It can't flag a plant with a fresh violation as a repeat severe-violation offender, can't score it, can't benchmark it against its industry, can't watch EPA and OSHA alongside it, and can't alert you when a supplier's status changes. That monitored, scored, three-agency view is the entire product.
How often is the data refreshed?
FDA enforcement data refreshes weekly (from the FDA Data Dashboard). EPA ECHO and OSHA establishment data refresh weekly. Facility risk scores recompute nightly so any new violation is reflected the next morning.
How is the facility risk score calculated?
The 0–100 score combines four sub-scores: violation frequency (3-year), severity weight (willful, repeat, failure-to-abate count more), recurrence (repeat severe-violation flag), and trend (improving vs. deteriorating). Higher tiers expose the sub-score breakdown.
Can I monitor specific facilities?
Yes — vendor watchlist monitoring is included on the Supply Chain plan. Upload your vendor list (or filter by NAICS + geography) and receive webhook alerts the moment a new inspection or violation is recorded.
Is API access included?
API access is available on Investor plans and above. You receive a dedicated API key with per-plan rate limits and full OpenAPI docs in your dashboard.
Is there a free trial?
Yes. Every plan includes a 7-day free trial with full access to the features in your tier.
